Rationale and Background Fak Tha Hospital opened the Warfarin Clinic in the year 2016. Warfarin is a high-risk medication used in the treatment of cardiovascular diseases, characterized by its narrow therapeutic index, making it a high-alert drug. Complex dosing adjustments and patient adherence issues have led to drug-related problems (DRPs). Based on data tracking in 2021 and 2022, medication adherence was found to be 67.67% and 72.03%, respectively. INR values indicate treatment goals, with %TTR reflecting overall treatment quality. Higher %TTR indicates better overall treatment quality. In 2021 and 2022, %TTR was 46.17% and 49.74%, respectively, which did not meet the target (>65%). This innovation aims to ensure safe Warfarin use, maintaining patients within their Target INR continuously, without adverse effects.

Results of Innovation 1. Time in Therapeutic Range (%TTR) increased from 46.17% to 61.23% (2021-2024) 2. No DRPs requiring patient monitoring in the ward 3. Patients showed increased medication adherence from 67.67% to 100% (2021-2024)
